The Palliative Care Bridge is a state-wide, palliative care education program coordinated and delivered by the HammondCare consortium, comprising HammondCare, Sacred Heart Health Service and Calvary Healthcare Sydney.
HammondCare is an independent Christian charity specialising in palliative care, dementia and aged care, rehabilitation and older persons' mental health. HammondCare offers hospital care, residential care and home care services throughout NSW. HammondCare is passionate about improving quality of life for people in need and has a particular commitment to people who are financially disadvantaged.
Sacred Heart is a leading provider of palliative care and rehabilitation services. Operating as part of St Vincents & Mater Health Sydney and managed by St Vincent’s Hospital, Sacred Heart is an Affiliated Health Organisation. Sacred Heart offers specialist palliative care and rehabilitation to inpatients and outpatients. Inspired by the values of the Sisters of Charity, our mission is to bring the healing ministry of Christ to all we serve.
Calvary Health Care Sydney, a service of the Little Company of Mary Health Care group, is an Affiliated Healthcare Organisation specialising in palliative care, rehabilitation and aged care. Calvary offers inpatient, day-only and integrated community based services in the St George and Sutherland local council areas.
