Geriatric Rapid Acute Care Evaluation
GRACE
The Aged Care Rapid Response Team in the Hornsby, Ku-rin-gai area/Upper North Shore is known as GRACE: Geriatric Rapid Acute Care Evaluation. It is a Hospital Avoidance Service for Older People. This service aims to reduce avoidable Emergency department presentation and Hospital admissions. It provides rapid assessment for older patients in their own homes including Residential Aged Care Homes who are experiencing new medical and/or functional decline.
